When To Stay Home: When deciding whether to keep your child home due to illness, these tips can help!
Immunization Information: According to Texas State Law, all students must present a current immunization record or up-to-date Immunization Exemption Affidavit. If your student has received new immunizations within the last year, please submit an updated record in person or by email. Exemption Affidavits must be renewed every two years, notarized, and the original paper copy submitted to the school nurse.
Medication at School: For daily prescription medication administration, please complete the Prescription Medication Administration Form and return to school in person or by email to Brynn Mereness.
Please bring all medication in the original pharmacy container with label intact before the first day of school.
During enrollment, parents had the opportunity to give consent for administration of common over-the-counter medications. These medications are provided by LCS and available to students on an as-needed basis, according to our LCS Standing Orders.
Management of Chronic Conditions: If your student has a chronic health condition that may require management or medication at school, please provide documentation in the form of a management or action plan. Please print from the forms below and have your child's physician complete and sign. Some physicians prefer using their own forms. Either option is sufficient. Management plans may be submitted in person or by email to Brynn Mereness or your physician can FAX to 806.791.3569.
Please bring inhalers, epi-pens and other emergency medication to school in the original pharmacy packaging with label intact before the first day of school.
