Important Health Information for the Start of School

Immunization Records: If your student is new or received immunizations over the summer and the updated record was not uploaded during enrollment, please email the new record to the nurse before the first day of school.

Immunization Exemptions: Immunization Exemption Affidavits must be renewed every two years. Schools are required to keep the notarized original copy on file; no photocopies accepted.

Medication at School: If your child requires medication during the school day, a Medication Authorization Form must be completed. Please contact the nurse to arrange medication drop-off before the first day of school.

Medical Diagnoses: If your child has a medical diagnosis that may affect their school day (e.g., allergies, asthma, diabetes, etc.), please contact the nurse prior to the start of school to ensure proper care and planning.
